Admission and registration

All patients visiting the MCH for the first time must go to the  Admitting and Registration Office to register. For any appointment or hospital admission, please bring:

• The valid Quebec health insurance card of your child
• The hospital card of your child
• The list of your child’s medications, including over-the-counter medications, dietary supplements and natural products
• Your child’s immunization record

If your child does not have a valid Quebec health insurance card, please bring at least one of the following: 

• A valid health insurance card from another Canadian province or territory
• Proof of your private health insurance coverage
