Transition to adult healthcare
The Montreal Children’s Hospital hopes all teens feel empowered on their journey to adulthood. Our healthcare teams, along with the Pediatric-Adult Transition Hub – Espace de transition adulte-pédiatrique (PATH-ETAP), will support patients and their families.
