THIS SERVICE WILL BE AVAILABLE FROM JULY 7, 2025. This access point is available to patients who have a referral. If you do not have a referral, talk to your family doctor or another health care professional.

- You’ll get a confirmation email with a ticket number from [email protected].

What if I need help using the portal?
Call us at 514-934-1934 ext. 45605 – Available Monday to Friday, 8 a.m. to 8 p.m.
Can I make a follow-up appointment through the portal?
No, the MCH Referral Portal is for first-time appointments only. To schedule a follow-up, check the Clinics Directory and visit your clinic’s page for instructions.
Do I have to call the MUHC after submitting my application?
In most cases, it’s not necessary. Call us at 514 934-1934, ext. 45605 if you do not receive a confirmation e-mail OR if you no longer require an appointment.
IMPORTANT: If your health changes while you’re waiting, talk to your family doctor or call 8-1-1.
How will I know if MCH received my request?
You’ll get a confirmation email with a ticket number from: [email protected]
How will I know if my consultation was refused?
If your request is refused, you’ll receive an email from [email protected]. Your referring doctor will also be notified and asked to follow up with you.
What if I made a mistake in my submission?
No worries. Just call 514 934-1934, ext. 45605 and have your ticket number ready.
Can I fax my referral instead?
Fax is being phased out by the Quebec Ministry of Health and Social Services. We strongly recommend using the MCH Referral Portal — it gives you confirmation and a ticket number.
When will I get an appointment?
If your request is accepted, it will be reviewed and given a medical priority. You may have to wait up to six months for a call with your appointment date.
Someone from MCH called but didn’t leave a message — what now?
If you know which clinic it was, check the Clinics Directory to find their contact info. If you don’t know the name of the clinic, please call 514-412-4040.
I moved. Will this affect my appointment?
Changing your address won’t affect your appointment, but you should update your file in person at the Admitting Office, located on C RC.0036 (RC level near the Royal Victoria Hospital main entrance), and open Monday to Friday, 7 a.m. to 4 p.m.
Will I need any tests before my appointment?
If any tests are needed, someone from your clinic will contact you. If you don’t hear from anyone, no tests are required.
