The Intermediate-Complexity Coordination and Navigation Service (I-CCAN) assists children, youth and families in navigating the healthcare system and facilitates access to coordinated hospital-based services. I-CCAN is offered to patients with intermediate complex care needs who are followed by many different service providers.
Services
The I-CCAN service is designed to help families:
- Prioritize care delivery based on expressed needs.
- Harmonize the care offered by various services at the Montreal Children’s Hospital.
- Reduce the time dedicated to organizing hospital visits and decrease unnecessary travel to the hospital — for example, scheduling multiple medical appointments/tests on the same day when possible.
- Reduce delays in accessing needed care.
Each child and youth is assigned a care coordinator who serves as a point of contact to assist the family in coordinating, consolidating and simplifying hospital visits. The care coordinator organizes MCH medical visits, imaging, tests, therapies and/or procedures according to the family’s availability and preferences.
I-CCAN may also offer patients medical care consultations by our pediatricians via the I-CCAN clinic, as well as the support of a nurse practitioner/clinician. I-CCAN services offered to families will be determined at the initial needs assessment following enrollment and may be reassessed according to the evolution of the patient/family needs.

The I-CCAN team, composed of care coordinators, nurse practitioners, nurse clinicians and pediatricians, supports strategies such as family-centred proactive care planning and interdisciplinary collaborations to transform care delivery for children with medical complexity.
