Welcome to COCOE
COCOE aims at setting new standards for pediatric colorectal care in Canada and improving the lives of families and children living with anorectal malformations or Hirschsprung’s disease. Our mission is to:
- Offer outstanding, compassionate clinical care using a comprehensive and interdisciplinary approach
- Investigate and improve outcomes in pediatric colorectal diseases, focusing on patient and family experiences
- Provide outreach medical education for patients, families, and communities, training for healthcare practitioners and professional development programs

Our medical team discusses individual patient cases during multidisciplinary conferences to create a customized treatment plan that addresses each child’s unique needs. Our surgeons are accustomed to performing multiple, complex procedures at the same time. Parents and families work closely with our specialists to receive education about their child’s colorectal disorder, discuss diagnostic and treatment options and engage in a shared decision-making process.

Bowel Management Program
COCOE offers a comprehensive Bowel Management Program that provides customized treatment for children who experience constipation and/or fecal incontinence (the inability to control their bowels), such as previously operated anorectal and cloacal malformations or previously treated Hirschsprung’s disease. Our program uses a multidisciplinary approach, with access to the pediatric colorectal specialists needed to oversee your child’s care.
Urologic and gynecologic management for anorectal malformations
Our COCOE team understands that children with ARM may need on-going, coordinated treatment and care from specialists across multiple medical disciplines in order to maintain urologic and gynecologic health throughout their lifespan.

Pediatric Colorectal Fellowship
COCOE and the Division of Pediatric Surgery at Montreal Children’s Hospital now offer a Pediatric Colorectal Fellowship. This fellowship combines clinical and research fellowship aims to train participants in the comprehensive management of colorectal conditions using an interdisciplinary approach to care. The program addresses the growing demand for specialized care in pediatric colorectal disorders while providing advanced training opportunities for future leaders in the field.
Mission and training objectives
Mission
The goal of this Fellowship is to train individuals who will become experts in managing pediatric colorectal disorders, from newborns to adulthood. This fellowship will provide the foundation for an academic career in pediatric colorectal surgery and research.
Training objectives
The Training Objectives section of the Colorectal Fellowship at the Montreal Children’s Hospital outlines the comprehensive and specialized skills that fellows are expected to acquire during their training.
Educational hierarchy and supervision
The colorectal fellow will respect the educational hierarchy already in place, with the Pediatric General Surgery Fellows having the right of first refusal to all clinical activities. The colorectal fellow will not be a senior supervisor of residents. The colorectal fellow may either report directly to the attending pediatric surgeon or take call alongside both the attending and the pediatric general surgery fellow. In the latter case, the colorectal fellow works under the general surgery fellow in the educational and clinical hierarchy. An attending pediatric surgeon remains available on-call 24/7 to provide overall supervision and support, ensuring continuous coverage and appropriate oversight for the Colorectal Fellow’s responsibilities.
Activities
Clinical activities
The Fellowship offers participants the opportunity to gain subspecialized expertise in pediatric colorectal surgery via COCOE-related activities. It also allows trainees to gain substantial experience in pediatric general surgery through on-call activities.
COCOE activities
The Colorectal fellow will be expected to assist ongoing elective clinical activities. The clinical load will account for approximately 40% of the Colorectal fellow’s time and is broken down as follows, over a 28-day period.
On-Call activities
The on-call service will be a mandatory activity. The fellow will cover call for the Harvey E. Beardmore Division of Pediatric Surgery. The role of the on-call activities is for the fellow to maintain the level of training and adequate exposure to pediatric general surgery and to hone their technical and diagnostic skill set.
Research activities
Research projects depend on the fellow’s interest and prior knowledge and can include a range of methodologies available at the MUHC and MCH. Fellows are expected to spend a minimum of 60% of their time on research-related activities. Which may include clinical, translational, health services, and educational research.
Eligibility
Eligible candidates should be enrolled or have completed a general surgery residency program. Preference is given to candidates that have already secured sponsorship or funding for the fellowship. International applicants must have proof of completed their surgery training in their country of origin.
Applicants should:
- Be fully licensed physicians in good standing in their home country.
- Have current clinical responsibilities in the field that would be enhanced by the completion of this fellowship
- Have plans to return to their home country to develop a colorectal program there
- Possess the support of their home institution and demonstrate that they will be able to implement a program upon return.
- Have the potential for ongoing collaboration and engagement of fellow/institution with the COCOE team and Montreal Children’s Hospital
Preference is given to applicants who are self-funded.
